PRACTICE ALERT New Best Practices for Treating Preeclampsia By Mark Zakowski, MD Member, Preeclampsia Task Force, California Maternal Quality Care Collaborative, Chair, Legislation and Practice Affairs Division, California Society of Anesthesiologists, Chair, Education Committee, Society of Obstetric Anesthesia and Perinatology T he recommendations for the management of even long-familiar diseases, like preeclampsia, occasionally require major updates. The California Maternal Quality Care Collaborative (CMQCC) recently released the Preeclampsia Toolkit (www.cmqcc.org/ preeclampsia_toolkit ) - 158 pages of useful information, guidelines, algorithms, clinical pearls, and even an educational slide set. When the collaborative released its OB Hemorrhage Toolkit in 2010, the essence of the advisory, if not the exact wording, spread nationally and led to new standards of practice and improved patient outcomes. It's likely the same will occur with the Preeclampsia Toolkit, so you are advised to review the user-friendly materials. The Preeclampsia Task Force that developed the information and guidelines was a collaborative effort of the CMQCC, the California Department of Public Health, and the Maternal, Child and Adolescent Health Division of the Center for Family Health. The toolkit was developed over a two-year period. Background The maternal death rate in California nearly doubled, to 14 per 100,000 live births, between 1999 and 2008. The spectrum of hypertensive disorders of pregnancy remains a leading cause of peripartum maternal mortality, accounting for 17 percent of these deaths nationally. Preeclampsia also leads to premature birth, with concomitant neonatal morbidity and mortality.
