Midwinter Educational Interlude By Adrian W. Gelb, MBChB, CSA Educational Programs Chair F or decades, CSA has hosted a January educational meeting on Maui. The success of the program has demonstrated that a combination of excellent lectures, a dose of vitamin D, and some relaxation are the perfect amalgam to enhance education. This year's Winter Anesthesia Seminar was no exception. Although the event was held a little earlier than usual (Jan. 6-10), and at a new hotel, the Fairmont Kea Lani, CSA standards were maintained and the attendees enthusiastic. As usual, the program focused on clinical practice, with special emphasis on patient safety and improving perioperative outcomes. The speakers brought data and perspectives that covered obstetrics, pediatrics, trauma, obesity, neuroanesthesia and cardiac care. The five speakers demonstrated their mastery in the management of diverse patient groups. Dr. Rick Dutton spoke about hemostatic resuscitation of the trauma patient, covering not only the ABCs but also an in-depth review of fluid choice, based on his experiences at the Baltimore Shock Trauma Center. He also gave an excellent, very practical lecture on the perioperative management of the patient with obstructive sleep apnea. And, yes, two of his lectures did relate to his current position as Executive Director of the Anesthesia Quality Institute (AQI) - "Quality in Anesthesiology" and "Tales from the Anesthesia Incident Reporting System." Each was an entertaining and instructive tour de force on anesthetic misadventures and how to manage them. 58 | CSA Bulletin Dr. Joy Hawkins covered anesthesia for obstetric care and for the pregnant patient having non-obstetric surgery. Her lecture on obstetric emergencies dovetailed nicely with Dr. Dutton's on hemostatic resuscitation, as she spoke about management of obstetric hemorrhage and other emergencies. Other lectures presented the current views on anesthetic agents and their safety in pregnancy, management of the traumatized pregnant patient, neonatal resuscitation, and the clinical implications of relevant clinical practice guidelines. A combination of excellent lectures, a dose of vitamin D, and some relaxation are the perfect amalgam to enhance education. Discussion of the perioperative assessment and management of patients with cardiorespiratory disease was the mandate of Dr. Scott Beattie from Toronto. He reviewed management of the patient with a recent coronary stent, appropriate use of cardiovascular drugs, and preoperative testing of the cardiorespiratory systems. Much of the evidence he presented was from his own work using the large perioperative database in Toronto.
