Unfortunately, I only was taught strip crowns in residency. I have been trying to arrange a time when I can watch my boss do the preveneered or the zirconias, but it looks like it will be at least another month or two before we can get our schedules to align. I called NuSmile and they are sending me an instructional DVD, but in the meantime I am anxious to learn tips and tricks in advance from those with experience. I have read most of the old threads on this topic, but just curious if anyone can walk me through the typical issues that arise and the actual prep itself. For strip crowns I am used to 1.5mm-2mm of incisal reduction with a football bur, .5mm (ish) interproximal reduction with a skinny diamond burr to break interproximal contacts, a little circumferential reduction (just barely dinging up the gingiva if possible) and sometimes a bit of facial reduction (.25mm-.5mm), and a bit of cingulum reduction (.5mm-1mm), then I fit the strip crown and do the rest of the procedure. ■ SEP 9 2014 Pedo911 Member Since: 03/12/05 Post: 3 of 18 To be honest I can't tell you how many millimeters I actually reduce. For whoever is wondering, I haven't done the RVSSC's in over a year since switching to ZR. You will definitely need to reduce more than strip crowns, especially on the lingual cingulum and facial. Occasionally I reduce even more circumferentially if I want a smaller crown for better esthetics. ■ SEP 9 2014 Steven L Member Since: 05/03/06 Post: 4 of 18 Have been doing the RVSSC NuSmiles for years and the zirconia now for a year. I still use both and feel some cases lend themselves to one over the other-if only for the reason that the RVSSC can come as "short" and, to my knowledge, the zirconia only come in one size with respect to height, which most closely resembles the height of the "regular" RVSSC. After hundreds of these I still struggle and don't feel that they really lend themselves to ever being a perfect solution. That being said, one thing that totally improved my success and made me much better with them was watching the video. I had been doing them for quite some time based on training in residency and became frustrated that I wasn't getting better with them. I watched the video and then did my following cases exactly like the video showed and had much more success from the get go. So watch the video. Also, I am stealing this from someone here on DT but can't remember who to give credit. If doing DEFG, prep a central first, then the opposite lateral (example, prep F and then D or E and then G) and then find the crown that fits these. Then, you know relatively for certain that the incisal length is pretty accurate because you can compare to the unprepped central and laterals. Also, this allows you to ensure that the M-D width is OK and that you should be able to subsequently prep the other central and lateral to accept the same size crowns. I made the horrendous (in my hands) mistake forever of prepping all four at once.
