for the excess of material. I didn't inject the composite through the holes. ■

I see. that's quite a neat trick. did you section the two semi-direct onlays with the round disc?
It must be tricky to get the contact points correct? ■

Beautiful work. Thank you for posting. How did you bond the restorations and what do you
think are the chances the teeth need endo therapy within the next 5 years? I know that's a difficult
or even worthless question, but patients want predictability after chair time... How do you discuss
what you've done and the prognosis? ■

So is that Teflon tape over preps? Looks like you established the contacts first directly and
then topped it off with the onlays, correct? How do you section them apart without affecting the
embrasures too much? Thanks Jack ■

The index is made using a clear silicone such as Memosil, or Affinity Crystal Clear to name
a few materials. Ideally you want the lab to make them as they can be pressurised therefore they
are more accurate. I prefer the material to have Shore hardness of 75 so that there is some rigidity.
If you want to make them at chairside, use a small index tray. Make sure you drill the holes
from the inside of the index.
Well that's how I do it. Have a look at Vailati's 3 step technique. ■

Neat technique. I've done the same indirect on quick stone models of the preps.

I charged about a third of the fee I ask for emax onlays but, no need for a second appointment,
no individual impression, easier preparation, simple bonding protocol, no lab fee (I did a quick
wax up myself). This patient had no reimbursement for indirect restorations.
I applied regofix over the wax up and i relined the thus obtained matrix a second time with
the same material.
I used a scalpel to make a small cut and than I snapped it in two. It didn't work out as planned
as you can see on the x-ray; the emergence profile of the second molar is not ideal.
