It is more than just belief Bill. A systematic review by Garaico-Pazmino et al
(2014) found a significant negative association between the crown to implant ratio
and marginal bone loss - the greater the ratio the better the marginal bone level. It
appears that a high crown to implant ratio could actually provide a protective effect
on implant marginal bone level. This finding is consistent with an evaluation of periimplant bone density around implants of different lengths (Sahrmann P et al, 2017).
Bone around short implants after 3 years had higher mineralization on radiographs
but this was not observed around long implants. Bone remodels in response to forces
placed upon it (Wolffs Law). Under overload conditions bone turnover actually
increases resulting in bone gain (Frost, 2000). n

Bill, I've placed 8 and 6mm long implants and they work fine. Do you advise
restoring dentist of any precautions when restoring 6mm implants? n
Bill and Craig - thanks so much for these posts. You are advancing the practice of
implantology tremendously. Whether by luck or by genius, the people at Bicon have
been blazing this trail for a few decades. And whether by luck or by genius I fell into
their school of thought and training. There are kick-butt dentists like Shadi Daher,
Ranier Urdanetta, and Draseo Speratti proving day in and day out that, at least with
the Bicon implant, short is not only good, it is better.
We need to acknowledge as a profession that when placing a "short" implant or
an implant with a 3:1 crown to implant ratio, that that is not only acceptable but
normative. There is no justification for the nerve repositioning and lateral sinus lifts
that we sought in the past to accommodate 10 and 15 and 18mm implant lengths in
light of current research and practice.

8mm long implants are NOT short - so why would you restore them any
differently.
6mm long implants ARE described as "short" in the CURRENT literature, but
I don't treat them any differently when I restore them. n

Bill, beautiful work as always
The fact you are NOT grafting and placing in native bone also plays a role, from
what I've read. The histological profile in native bone in relation to osteocytes is
obviously normal. The osteocytes induce osseous remodelling n relation to loading
forces. So these results do not surprise me at all.
Osteocyte profile is why I'm a fan of autogenous grafting, specifically Khoury
plate. n
