Another Challenging Class 3 Case
Introduction:
This is a 13.5 year old boy who I treated for phase I with an RPE plus protraction headgear
and upper 2x4 when he was 8 years old.
In phase I, I feel I got a nice result with ideal overjet and OB, then I made him an upper
retainer, then they never showed
up for another appointment until
recently-four years later.
He is clearly Class III, but
presents as more maxillary
retrognathic than mandibular
prognathic and I'm trying to

Conclusion:
While I feel this is ideally a surgical case, I know this family and know they will likely
refuse any surgery recommendations. Would any of you attempt to do anything now or
consider any further camouflage treatment, or would you just say we need to wait for growth
to stop and do surgery?

The maxilla is very deficient in the AP and vertical, and both upper and lower incisors are already
very compensated, perhaps at the outer limit of camouflage. Growth direction seems more vertical
in the progress ceph, although facial thirds appear reasonable. I would definitely not attempt further
camouflage and suggest the parents wait for cessation of growth and jaw surgery to resolve the skeletal discrepancy. Given that he appears maxillary deficient (as opposed to mandibular prognathic),
you could consider decompensating the arches right away, (with extraction of upper bicuspids) in
anticipation of surgery 14-18 months down the line (after comparison of serial cephs of course). ■
