Most had braces when they were younger but were never told to wear retainers for more than a couple of years. To them, their teeth "look straight." And indeed, they often look pretty good. But not fabulous and natural. Maybe the communication gap regarding lifetime retainer wear centers on the difference between teeth looking pretty good or fabulous. Did you ever see an old patient in town with her mom and the mom is excited to show you how great her daughter's teeth look, and she smiles and you see the slight relapse? Retainer Protocol Reasonable Approach to Lifetime Retention Extract Maxillary When I was teaching at Harvard, there was an instructor who I personally thought was one of the greatest clinical orthos I had ever seen, but what he made the patients do as far as work to get the spectacular result was ridiculous. I always thought to myself, "He really made the patients earn that result." To me that is the old attitude toward orthodontics. Kind of like Andy Haas talking about tying HGs in older teens. Make 'em work for it! You sound the same in talking about retainers. As a businessman who depends on patient referrals for a large part of his new patient flow, I want the best possible result for as long as possible. I think that you achieve that with fi xed retainers, not removable retainers. I can't count the number of parents who have brought their 3> in or have written on my FB page that they had braces 20 years ago or so with me and their kids teeth still look great. When it is a mom in the office with her child, I ask about the fi xed lower retainer and frequently they show me that it is still there. When I take off the lower retainer or if I was using a removable, I would IPR the lower incisors with a very thin wheel to enhance the effect of the retainer, à la Lee Boese. n APR 5 2015 Goodmany2k Member Since: 11/12/09 Post: 3 of 62 I have a great technique for bonded splint. Ortho-flextech chain. Soak precut length in unfi lled bonding agent. Etch and prime teeth. Put large bead of composite in canines only. Take chain and lay it in place and it will be held up by the bead on each canine and be tacky and hold on 2-2 while you position chain. Cure and then bead 2-2. Über-passive technique. Many floss-pulled wires cause movement . . . sometimes severe. And yet . . . I still think it is not for everyone (I mean that in several ways). But tell me more about the light stripping of the lower anteriors after alignment. n 5> APR 5 2015 Str8edge Member Since: 07/17/11 Post: 4 & 6 of 62 This by definition is not great because it is bonded to all the lower anteriors-this is an impossible hygiene situation for patients and the reason that GPs and hygienists hate fixed retainers. We have to live in the real world and realize that many patients are not going to do what we instruct them to do (i.e., brush their teeth, wear elastics, wear retainers)-so if you place a well-adapted lower fi xed bonded only to the L3s and have a good occlusion, you will see very minimal relapse.
