Even with the Xn of upper 4s, the incisors will procline, especially with continuous arch
mechanics. And 4s will def. be faster in terms of the speed of alignment. ■

The more I look at this case, the more I like U4 and L2s to be extracted. It just seems like
that is the natural progression of the teeth. I can see this case looking very good in 12 months. ■

I'd be awfully tempted to go U4s, L2s here. Only hangup is the size and shade of the L3s- a
little yellow. ■

It is a tempting tx plan, but I worry as you mentioned of the Bolton discrepancy this would
cause. I believe it would require significant mandibular IPR. ■

Do a diagnostic wax-up and see what it looks like. Function, stability, aesthetics ... sometimes
you can only choose two. ■

We have a Trios scanner; maybe I will see if I can do it on there. ■

Send patient to OMFS for ext U/L4s and 3rds/supernumerary as the surgeon sees fit. Nance
on upper to ensure molars stay. Sectional mechanics on the lower until both L3s are retracted
(this would include banding L7s from Day 1).
Once in large upper base-wire (19x25 SS), piggy back U3s down with 12 or 14 NiTi. Bond
L2-2 once 3s are retracted. Torque lower incisors as needed in rectangular wire. U2s look small; may
need to run Bolton and do some lower IPR, too. Don't complicate things. Go for the slam dunk. ■

How 'bout just removing the lateral? With elastics or Forsus, move the dentition on lower
forward. As much as you want to get the root straight, it won't happen. I also wish to ext. the
upper 4s, just to be clear. ■

I don't see that (just the one lower lateral) working. Even extracting both laterals (and U4s)
will likely leave me with insufficient OJ due to small U2s, and extraction of L2s and U4s itself
leaves a Bolton discrepancy. ■
