I wrote a similar column in the spring of 2012 and it's as relevant
today as it was then. With a few minor changes I am reprinting it.

hese two quotes by a great American, I think, describe the
physicians' situation in the United States at this time. The
first quote is what we all feel like, we are at the end of our
rope; and the second quote describes that we need to be honest
about why we are in this mess.

We are part of a great profession. The need for physicians is
a given, but how come we have no control over our own destiny?
We are harassed by the insurance companies; we are pushed to our
limit with certifications and re-certifications; we are looked at by
multiple organizations as to whether we are doing a good job. If
these measures make us better doctors and improve the quality
of care in this country then I have no problem with them. My
biggest concern and as I said, we need to look at it honestly, is
how come our standard of living is dropping? We have difficulty
meeting our obligations both at home and in our offices. We are
literally living from moment to moment and being buried under
regulations that just continue to increase. From ICD-9 codes
which number 13,000 to ICD-10 codes which number 68,000.
If after 25-30 years of total education, including residency
and fellowships, we can barely make ends meet, is it worth all the
struggle and expense? Are we doing something wrong that we are
not successful?
SERMO, one of the world's largest internet servers for
physicians, took a poll which showed there was dissatisfaction by
physicians on just about every aspect of medicine: 59% felt that the
quality of medicine is dropping in the US; 62% were pessimistic
about the future; 77% felt that struggle with payers takes away
