but with a mindset of "leftover medicine"-perhaps due to prematurely assuming that there would never be sufficient resources to
offer antiretroviral treatment (ART) to Africa, and as a result, there
would be little impact on changing the trajectory of the pandemic.
The development model lamented the setback to economic progress
and tended to respond by advocating for personal responsibility
(e.g., abstinence or monogamy), which could easily slide into victim
blaming ("if only these people were not so backward..."). The social
justice model called attention to the injustice of failing to provide
known effective treatment and advocated for universal access to
ART. Eventually, the moral dissonance of having one standard for
the developed world and a different standard for Africa became
intolerable, and programs like PEPFAR and the UN Global Fund
began to extend ART to millions in Africa. In Malawi, in the decade
after ART first became available, HIV deaths fell by 51 percent and
new infections by 65%. It turns out that treatment is by far the most
effective form of prevention.

For me, in 2018, the term "medical missions" carries too much
of the connotation of the charity model, reflecting an underlying
assumption that "we" have something "they" need but can't provide,
as opposed to a commitment to listen to and learn from those we
seek to serve. Although undeniably the source of much good work
in its day, it is now time to move beyond this to a vision of global
health equity, rooted in universal human rights and a preferential
option for the poor.6 In the words of the PIH Mission Statement,
"our mission is both medical and moral. It is based on solidarity,
rather than charity alone."7
