How would we improve the POLST program?
All patients could benefit from a secure internet website
housing their latest advance directive. In an analogy to the
present Pennsylvania Department of Health Prescription Drug
Monitoring Program (PDMP), POLST forms and up-to-date
advanced directives should be available for download to any
health care facility and patient transport services without relying
on patients' personal copies.
This "cloud" designated for advance directives has been
implemented for Medicare patients in Oregon. These patients
were more likely to have their end-of-life care preferences
followed, including choices for dying at home, limiting ICU
admission and enrolling in hospice care, compared with
patients living in neighboring Washington state or the rest of
the United States. *
