ENGAGEMENT STRATEGY
TO HELP CARE MANAGERS
EDUCATE PATIENTS
AT THE RIGHT TIME-

BEFORE IT'S TOO LATE

T

he best clinical interventions
are the ones that happen
before a patient gets into
trouble and needs emergency
care. However, without timely,
accurate insight into a patient's condition,
it's just a guessing game as to which patient
needs your care today. Data-driven, daily
patient engagement is a strategy that helps
care management teams deliver optimal care
before a serious health event occurs.
WHAT IS PATIENT ENGAGEMENT?
Across the industry, a variety of definitions
for patient engagement have been offered,
suggesting that it means:

* An active partner working to improve
health and healthcare systems1
* An informed, educated, involved partner2
* One who accesses and uses his or her own
electronic healthcare record3
* The actions we take to support our health
and benefit from healthcare4
* Improving patient experience and satisfaction with care5
While each of the above definitions
has merit, they all fall short because they
lack specificity and measurability. A successful definition of patient engagement
should consider:
1. Measurable outcomes (Can engagement
be quantified?)

2. Measurable processes (Is there a method
to support and increase engagement?)
3. Measurable satisfaction (Does the
engagement align with the support that
patients say they want?)
Instead of saying that patient engagement
is something that we'll know when we see
it, the actionable and measurable definition of patient engagement is the regular,
consistent, and persistent engagement of a
patient in his or her self-care.
Patient engagement is an ongoing process, not a one-time event-and it's also a
two-way street.

built upon several factors:
