emotional or spiritual issues. I see palliative care benefitting all stakeholders-obviously the patient, but also the patient's family and primary doctors. If palliative care weren't in the picture, then a number of issues that seriously ill patients might confront because of their illness and side effects of their treatments will fall on the shoulders of their primary caretaker and primary physician. But if palliative care is part of the treatment plan, then you have this extra layer of support that makes the treating physician's role much more effective because it's more focused on the treatment. I also would note that a key benefit often underappreciated and under-reported is that palliative care, when well provided, should help improve the patient's informeddecision making. You have a palliative care clinician, and perhaps a team, that develops a relationship with the patient and family, and may know more about the patient holistically than does the treating doctor. Because of the relationship that develops, the climate for informed-decision making, with all issues taken into account, is simply better. Leonard Berry - They're closely associated, and on parallel tracks in the sense that the more rapidly value-based payment systems develop, the more rapidly palliative care will grow. The payment reform is going to be correlated with a growth in palliative care and hospice care. If, for example, you were a wholly capitated system such as a Kaiser Permanente, why would you not want to invest significantly in broadening your reach of palliative care? We know from the empirical literature, palliative care that reaches more seriously ill patients more quickly in the illness cycle should reduce more expensive types of utilization, such as hospitalizations and emergency room visits. If you're being paid on a value-based basis, such as capitation, where you do better financially if you keep people healthy and out of the hospital, as opposed to fee-for-service, where you do better financially if you put people in the hospital, then the palliative care business line should benefit, as value-based purchasing grows. And in branding the palliative care service, we should not use the words death or dying. It's not about that. It's about living as well as you can for as long as you can, and doing the things that are important to the patient. And if you're being treated in a way that's toxic, which makes you sick, which causes suffering, then you're going against the grain of what palliative care can do for the quality of life. So I think palliative care is very supportive of quality of life for seriously ill people-and those enduring chronic illness with life-altering ramifications.
