the reverse side of the document. Then the original is given to the patient or the patient's surrogate, and multiple copies are made for those involved, as well as for any setting to which the patient is admitted. Copies are as legal as the original, and in the event multiple forms exist-the one with the most current date has authority. If a person has competing documents (other advance directives) stating different wishes, the most current is the one followed. There is a wealth of information freely available to download and links to statespecific POLST paradigm authorized organizations, which can be found at the following website: www.polst.org. Advance care planning is a complex, multi-faceted, emotionally tinged issue. The ideal scenario is as follows: 1.) Better education of patients, physicians, and healthcare professionals regarding endof-life issues; 2.) Discussion of illness and likely prognosis between patients, families, and physicians, facilitated by trained healthcare professionals; and 3.) Cooperative "Rich conversations are ones where the primary focus is to hear from patients and/or their surrogate the thoughts, feelings, concerns, and goals for treatment in light of the fact they have now been identified as having a serious medical condition that could be life-threatening." relationship in advanced care planning between patients, families and physicians as part of a normal course of treatment (not in a crisis) in the clinic or physician office. In closing, it is important to know the default response. When there are no decisions made, no surrogates identified and a patient is incapacitated, the default response is do everything necessary to keep a person alive. The default response is the most conservative one and one that reduces the liability on all involved. This may not be what you, your family or your healthcare providers feel is the best approach in your situation. So, if it matters to you, then the best way to ensure that your wishes, desires and goals are followed is to have a "rich conversation" with those who matter. ■ Michael Demoratz, PhD, LCSW, CCM, is an Aging Life Care Professional and Subject Matter Expert in End of Life Care, a current board member for the Coalition for Compassionate Care of California, an elected commissioner for the Commission for Case Manager Certification.
