Journals

Hot topics from this
month's AACN journal
Oxygen administration is often
assumed to be required for all acutely or
critically ill patients. However, injured body
tissues and cells respond both beneficially
and adversely to delivery of supplemental
oxygen. This article describes the balance
between maintaining adequate oxygen
for cellular metabolism and avoiding the
formation of reactive oxygen species.
Specific patient populations include those
with myocardial infarction, stroke, acute
respiratory distress syndrome, trauma and
traumatic brain injury. Nurses need to be
aware of the limited benefit and possible
harm of supplemental oxygen, avoid inappropriate use and identify specific targets
for titrating or discontinuing oxygen
therapy.
(Siela, CCN, August 2017)

Nurse-led quality improvement projects not only improve patient outcomes,
they have a significant financial impact
on hospitals and engage nurses in professional development.
AACN's Clinical Scene
Investigator Academy
provided training and
mentorship to teams
of nurses at hospitals
in seven regions as
they implemented a
change management
project to improve
care on their unit.
Evaluation of the
projects' outcomes
showed sustained
practice change in
most respondents to
a one-year follow-up
survey, cost savings
for the participating
hospitals and a sense
of empowerment for the nurses involved.
(Lacey, CCN, August 2017)

Patients with heart failure who are
admitted for noncardiac conditions are at
risk for developing acute decompensation
during their hospital stay. Such patients
may have heart failure
with preserved ejection
fraction, in which case
biomarkers such as B-type
natriuretic peptide may
help identify the risk for
acute decompensation.
Recognizing and intervening to manage heart
failure symptoms can
have a profound impact, as
these patients often experience frequent hospital
admissions and declining
quality of life. The nurse's
role includes patient
education to promote
self-management and
participation in interprofessional teams that offer
comprehensive support.
