Student Associates: Be sure to use the special Student Associate Registration Form on pages 73–74 Because Congress features more than 140 sessions in addition to a range of networking opportunities, career development programs and other special events, general registrations cannot be taken over the phone. Basic Registration Fee The basic Congress registration fee is $1,195 for ACHE members and $1,395 for nonmembers. The registration fee includes the following: • Numbered seminars totaling 9 ACHE Face-to-Face Education credits • Opening Session and one Hot Topic Session or two Hot Topic Sessions, each worth 1.5 ACHE Face-to-Face Education credits • One Congress reception (Monday or Wednesday evening) • One luncheon • Continental breakfasts Monday through Thursday Due to the heavy volume of Congress registrations, it takes up to 10 business days to process your paper registration; online registrations are processed immediately. Please be patient; your registration will be con rmed by email within two weeks after your form is received. Call ACHE’s Customer Service Center at (312) 424-9400 if you do not receive a con rmation. When completing the registration form, keep the following information in mind: Seminar/Program Selection When selecting your seminars, please do not select two seminars with the same number. For example, 23A is the same program as 23B. In addition, be aware of time con icts as you register for various events. The registration checklist on pages 63–67 can help you plan your schedule. Please note: Every effort is made to meet your rst choices for seminars, general sessions and receptions. In some cases, however, we may have to assign you to an alternate choice. Registering for Additional Programs You may earn more than 12 ACHE Face-to-Face Education credits by enrolling in additional seminars and programs and paying additional fees. To register for extra programs, see Sections 2–6 of the registration form. You may also purchase additional luncheons for $80 or an additional reception for $75. Additional reception and luncheon tickets may only be purchased on-site, as space permits. Hotel and Airline Ticket Information To help you make your hotel arrangements, ACHE has reserved a number of rooms at a reduced rate, which are available on a rst-come, rst-served basis until February 8, 2013. For more information about hotel arrangements, see page 60. ACHE has arranged reduced airfares with United Airlines. Go online to www.united.com and use the offer code ZNMC974504 to receive a 2 to 10 percent discount. All ticketing and booking fees will be waived with an online reservation. To make a reservation over the phone for a $25 service fee, call (800) 426-1122 and use the same offer code. ACHE reserves the right to substitute faculty and cancel or reschedule programs due to unforeseen events. For this reason, we advise against purchasing nonrefundable airline tickets. On-Site Registration Registrations that are received at ACHE by March 6, 2013, will be processed in advance of Congress. If your registration is not received by March 6, 2013, you must register on-site at Congress. While we encourage you to register early, on-site registration is available throughout Congress. For your convenience, remember: If your fully prepaid registration is received at ACHE by February 11, 2013, your registration materials and name badge will be mailed to you on February 25, 2013, allowing you to bypass the main Congress registration area on-site and simply pick up your portfolio of materials. For this reason, we encourage you to register early. Please note: Congress registrants with international mailing addresses must pick up their materials on-site. Cancellation Policy A written cancellation postmarked no later than February 22, 2013, entitles you to a refund (less a $100 processing fee, or $50 processing fee for the Student Track) or a full credit toward another ACHE educational event. After that date, all fees/credits toward other seminars are forfeited. You are encouraged to send a substitute if you cannot attend.
