our care coordination model (exhibit
3) provides a high-level overview of our
organization's approach to coordinated
care. When integrated effectively, these

the patient's navigation of diverse medical
and social services needed to receive appropriate and meaningful care. the aco clinical care coordinators tap their knowledge
of the healthcare system, communication
skills, clinical training, and patient advocacy
skills to build trustworthy relationships
with patients and primary care providers.
the aco has successfully integrated principles derived from the patient-centered
medical home model into the acute care
setting through clinical case management.
in support of the aco clinical care coordinators, we have leveraged information
technology resources currently available
in our system to identify gaps in care and
population health needs. During the development of the acos, we assessed available
population health management resources
