This invitation is intended to facilitate the participant’s travel and visa arrangements but does not extend financial support. To request a letter of invitation, please fill in the form online at http://www.ada.org/5079.aspx. Join the ADA as an Affiliate member and pay only US$75 to attend the 152nd Annual Session in Las Vegas instead of US$275 as a nonmember if you register by September 9!
