She says that she is interested in gaining more education about her medications and how to live a healthier lifestyle. She confides in you and says, “My younger brother had a heart attack two months ago, and I am now really worried about my health as I feel it is beginning to spiral out of control.” Her medical history includes hypertension (HTN) for 20 years, Type 2 diabetes mellitus for eight years, and dyslipidemia for five years. She is also perimenopausal and reports mild hot flashes occurring about one time each month that are relieved by cold showers. J.K. hesitantly states that she does not monitor her blood glucose levels regularly. She has two HDL LDL LDL HDL LDL HDL HDL HDL HDL LDL LDL LDL HDL children, a husband, works a desk job, and lives a sedentary lifestyle. She has smoked cigarettes for more than 30 years and has failed multiple quit attempts. J.K. denies drinking alcohol, saying, “I was told to stay away from that stuff when they told me I had a sugar problem.” She has a sporadic diet and admits to eating fast food three times a week on average and cooking a lot of red meat for her husband and two “growing” boys. J.K.’s current medications include atenolol 50 p.o. mg daily, lisinopril 10 p.o. mg daily, glipizide/metformin 5/500 p.o. mg twice daily with meals, aspirin 81 mg p.o. daily, simvastatin 40 mg p.o. daily, and one multivitamin p.o. daily. J.K. also tells you that she had the influenza vaccine last week during a health fair at work. She reports that she has been taking all of her medications on time since her brother’s hospitalization, but before that she often forgot to take them and would run out of refills all the time. Upon assessment you determine J.K.’s blood pressure is 140/78 mmHg, pulse=72, weight=168 lb., and height=5’1”(BMI=31.7 kg/m2—obese). Lab tests from one week prior to today’s visit yielded electrolytes, renal function, and liver function tests within normal limits. Her A1c was 8.8% and fasting blood glucose was 210 mg/dl. The lipid panel included a total cholesterol (TC) of 155 mg/dl, low-density lipoprotein cholesterol (LDL-C) of 92 mg/dl, high-density lipoprotein cholesterol (HDL-C) of 35 mg/dl, and triglycerides (TG) of 140 mg/dl. J.K. wants to know, “What do all of these numbers mean? Should I be worried about having a heart attack like my brother?” What would you say to J.K.?
