I am a physician who not only sees his patient from the neurology point of view, but also from a holistic perspective; so when I saw her, I was not as concerned about her epilepsy as I was about her expanding abdomen. I reviewed all of her records, examined her multiple times, and finally found that the abdominal distension was caused by severe hypothyroidism, which led to paralytic ileus. Still, the ultrasounds had reported normal. But after performing thyroid tests and concentrating on that differential, I discovered the underlying pathology. We treated her in the ICU, and she was doing much better when I left. This attests to the fact that doctors can really investigate the situation when they have the time.” What is his advice for helping to overcome any potential challenges? “Every facility has different guidelines, computer systems, paperwork, and communication methods between the primary care physicians and specialists. You also have to learn new names and see new faces. So, I always introduce myself, and usually within the next 72 hours, everyone knows that I am the locum tenens physician and will be at the hospital for a period of a few weeks.” He continues, “I also believe the coming years will be more challenging for us because the patient population is aging, which will result in many difficult-to-understand pathologies. In fact, I was able to publish two unique cases last year in the May and December editions of Clinical Neurology News.” According to Dr. Jaramillo, however, the benefits of locum tenens are the most significant, both professionally and personally. “The flexibility is especially rewarding. Once I complete my opportunity, I have time to take care of personal matters. And, although I am not preoccupied with money, the pay is always very good. Plus, we have the privilege to manage difficult clinical situations, often in practices where there are no neurologists, and those medical communities and patients love it when we can come in and provide this service.” As for the future, Dr. Jaramillo already has his calendar full. “I have an interesting long-term opportunity coming up in the Midwest, and at the end of the year, I will be providing holiday coverage for a large neurology group in the Mid-Atlantic. I am also planning to provide services for 2 to 3 weeks each in the South and Southwest. We, as neurologists, have many opportunities from which to choose.” LIMITLESS OPTIONS Neurologists interested in locum tenens will be at no loss to find a contract to fit their preferences, says Ms. Rosen.“Requests will continue to multiply, as we are obviously looking at an aging population, as well as increases in many of the conditions neurologists treat, such as multiple sclerosis and Parkinson’s disease. New and better therapies are also allowing neurologists to treat a wider patient population.
