Emergencies are going to occur, and you need a broad spectrum of training and experience. For example, you will have to intubate, run a code, manage a ventilator, interpret EKGs and X-rays, and perform procedures such as suturing and placing a plaster splint. Also, guests will sometimes injure themselves during on-shore excursions, and they will return to the ship needing care.” At times, he says, the only resources available will be what you have on the ship. “The preconceived notion is that we call a helicopter anytime there is a life-threatening emergency,” states Dr. Bradberry, “but that is far from the truth. The bottom-line reality is that the ship is often out of helicopter range due to limitations of the aircraft. At sea, emergency helicopter rescues are reserved primarily for life-threatening surgical emergencies, but short of that, most situations must be stabilized and initially managed on board.” Cruise ship medicine also requires good communication. “The ability to get along with others is essential,” he explains. “Patient satisfaction is crucial not only because it is good medicine, but also because the cruise industry is an intense customer service business. “Although shore side logistical support is available via satellite telephone, the on board medical team is often otherwise solely responsible for saving a life, start to finish. While at sea, for example, you cannot transfer a patient to cardiology services. You truly have to practice medicine, and this strengthens your confidence in your medical skills.” Of course, there are many personal benefits as well. “I met my wife on board,” notes Dr. Bradberry, “and I have made many friends. Plus, I have been to almost every Caribbean island, sailed European itineraries, and experienced Hawaii and the Panama Canal, which I probably would not have seen otherwise.” He adds, “As senior officer, you are at liberty to basically go anywhere on the ship. Social obligations—such as co-hosting a VIP table once a week with the captain— are quite enjoyable.
