As with many structural and enzymatic proteins in the myocardium, troponins—or their metabolites—are released into the vascular space in the presence of myocardial necrosis. Although all muscle fibers contain troponin, the isoforms of troponins I and T present in cardiac muscle are distinct from the skeletal muscle forms. As a result, measurement of these cardiac troponins is specific to cardiac injury. When troponin is mentioned in this review, the cardiac form (cTn) is implied. Three distinct forms of troponin make up the troponin complex: troponin I, T, and C (see Figure 1).
