Patient Care Endocrinology & Cardiology - December 2007 - (Page 12) Using the new AHA guidelines for preventing coronary heart disease in women CONTRIBUTORS JESSICA SALT, MD, Assistant Residency Program Director, Medical Sub-Internship Director, Department of Medicine, Thomas Jefferson University Hospital, Philadelphia, Pa. MATTHEW DECARO, MD, Director, Coronary Care Unit, Thomas Jefferson University Hospital, Philadelphia, Pa. C oronary heart disease (CHD) remains the leading cause of death in both men and women despite continued advances in treatment. It is responsible for 1 in every 2.8 deaths that occur in the United States and kills more Americans than cancer and diabetes combined. Unfortunately, the threat of CHD is particularly underappreciated in women. A 50-year-old woman without CHD has a 40% chance of developing the disease during her lifetime as compared with a 12.5% risk of developing breast cancer.1 Management of risk factors can play a key role in survival; a woman at age 50 with an optimal CHD risk profile has an expected survival of more than 39 additional years. In contrast, a woman with 2 or more major risk factors for CHD has an expected average survival of only 31 additional years. Unfortunately, the number of individuals with no major risk factors dropped from 42% in 1991 to 36% in 2001, presumably due to the increasing incidence of obesity with its attendant risks.3 Since risk factor modification can beneficially affect outcomes, this strategy and other preventative measures against CHD require increasing attention from patients and physicians alike. New guidelines On March 20th of this year, the American Heart Association (AHA) published updated guidelines for the prevention of cardiovascular 12 PATIENT CARE ENDOCRINOLOGY & CARDIOLOGY www.patientcareonline.com http://www.patientcareonline.com
