These may be derived via a ‘top down’ approach, where headline epidemiological data is used. However, there are alternative ways of deriving baseline figures of likely patient numbers. For example, pharma companies are increasingly recognising the value of longitudinal patient-level data as a critical starting point, which can be projected to the population in question. BIA should be performed by pharmas for the time horizon of most relevance to the budget holder. This should accord with the budgeting process of the health system, which is usually annual. Such a framework should allow, however, for calculating shorter and longer time horizons to provide more complete information of the budgetary consequences. Finally, as there is considerable uncertainty in a BIA, a single ‘best estimate’ is not a sufficient outcome. Instead, a range of results to reflect the plausible spectrum of possibilities that the budget holder will face should always be provided. As BIA plays an increasing role alongside CEA in the comprehensive economic evaluation of a new technology, the development of a strong, transparent analytical framework built on best practice principles will be critical to informing 6 those who manage and plan for healthcare budgets (administrators of national or regional healthcare programmes, private insurance plans, and healthcare delivery organizations and employers who pay for employee health benefits), as well as meeting their diverse and highly specific evidentiary needs for data, methods and reporting. ■ About the Author Michael Aristides is principal, Health Economics and Outcomes Research, at IMS Health.
