Not being able to access such staff expeditiously is emerging as one of the largest threats to drug development programmes, not least in the reduced ability to react and deliver expertise to deal quickly with an opportunity or a problem — or even simply deal with planned capacity issues — as the programmes go through their various stages. Pharma has previously coped with this by shuffling staff around, but it no longer has this flexibility. This need to react quickly and effectively will increase as we enter the biologics era, but the option of investing in permanent staff for single or intermittent issues is both costly and takes time to achieve. Creating reliable systems that provide such expertise, while maintaining continuity within the company and lowering the overall costs of servicing the smaller numbers of drugs, is now critical. Most mainstream pharmaceutical companies are investing in plans for outsourcing core technologies and knowledge-based expertise, following on from the relatively successful outsourcing of more basic, routine tasks such as data entry and monitoring. They have seen how biotechs, which have never been able to afford big expert groups in-house, have had to rely heavily on external expertise or collaborations, and now want to outsource/insource up to 50% of their high- and medium-level activities. The critical issue is how to do that in a structured way, while maintaining the quality and skills development of the individuals concerned, and keeping critical core knowledge and continuity in-house. Even large CROs struggle to provide such expertise as it is incompatible with their business model. Currently, it is only accessible externally through small consultancy groups, freelancers or interims in an uncoordinated, usually un-integrated manner, and it can be difficult to change direction in a timely or cost-effective fashion.
