He said that surely I’d like to have ‘a good sleep during the operation, and to wake up at the end.’ I don’t think he would have done any harm had I not paid — although who knows? All those in the ward paid him.” Mrs Kulikova’s operation was successful and the operating surgeon came to her straight after theatre: “I was just coming round, barely able to open my eyes, when he told me that it had been an ‘extremely difficult operation’ in such a convincing voice as though he had risked his own life to operate on me — I felt compelled to pay a further 10000 rubles (£222) based on what others in the ward had given.” This is by no means an isolated case — in a poll of 2634 people asked to rate the healthcare system, and reported in September by the Russian daily newspaper Izvestia, 80.4% were either wholly or fairly unsatisfied with their experiences. Recent data from Transparency International’s 2008 Global Corruption Report ranked Russia alongside Gambia, Indonesia and Togo as joint 143rd out of 180 countries on the Corruption Perception Index, with Denmark, Finland and New Zealand ranked joint first as the least corrupt economies. With the planned introduction of mandatory health insurance in Russia, the scope for out-of-pocket payments in the healthcare system should be reduced.
