Association Addresses Needs of Young Adults With Diabetes T he transition years, between ages 18 and 30, are a highly mobile and often chaotic time for the emerging adult. Going off to college or finding a job presents numerous life and developmental challenges for this population. Young adults with diabetes face the added challenges of beginning to self-manage their condition and changing from a familiar pediatric care team to a new adult team. There is little guidance available for health care providers working with transitioning patients. To help fill this gap, the American Diabetes Association developed the new position statement “Diabetes Care for Emerging Adults: Recommendations for Transition from Pediatric to Adult Diabetes Care Systems.” The statement, published in the November issue of Diabetes Care, is available online at care.diabetesjour nals.org/content/34/11/2477.full, and its recommendations have been incorporated into ADA’s “Standards of Medical Care in Diabetes—2012,” available at care.diabetesjournals.org/con tent/35/Supplement_1/S11.full. The position statement provides a framework for health care delivery during the transition period that will help health care providers understand the needs of this unique population and help these emerging adults 4 experience a smoother transition from pediatric to adult care. “The goal is to ensure that emerging adults avoid gaps in care and a deterioration in health status during the transition period,” stated Shana Funk, ADA’s director of youth and family strategic planning. “Health care professionals can use the guidance in the position statement to assess their practice and see whether there are additional steps they can take to contribute to a seamless transition for their young-adult patients with diabetes.” Looking to the future, the position statement calls for ongoing and expanded research initiatives addressing this group, including the continued training and identification of adult providers versed not only in the medical care but also in the psychosocial needs of the young adult with diabetes. The next decade should see the emergence of evidence-based strategies that support best practices for the growing numbers of young adults with type 1 and type 2 diabetes who will be making this important transition, the statement says in its conclusion.
