Lin W ■ ith the recent emphasis on health care reform and comparative effectiveness research, intervention studies are urgently needed and federal funding for comparing interventions will be forthcoming. The good news is that occupational therapy researchers are already conducting clinical trials— here is a sample of recent studies. Marjorie McClure et al. reported on a randomized controlled trial of the Breast Cancer Recovery Program for 16 women with breast cancer–related lymphedema.1 Compared to controls who received standard care, the treatment group (who received an exercise/relaxation program) demonstrated significant improvements such as better arm flexibility, quality of life, mood at 3 months, weight loss, and bioimpedance. ■ Anne Watson and her colleagues investigated the effect of assistive technology (AT) provided by a multidisciplinary team in a public school system.2 Using a repeated measures quasi-experimental design, they found that 13 students in special education who received AT to address academic and communication goals showed significant improvement on individualized education program goals, compared to other interventions. These results may support the use of AT in special education by occupational therapists, as delineated by the Individuals with Disabilities Education Act of 2004. ■ Tim Shurtleff and his colleagues examined whether a 12-week hippotherapy (therapy using a horse) intervention improves head or trunk stability and upper-extremity reaching in 11 children with spastic diplegia cerebral palsy.3 Using video motion capture with surface markers, significant improvements with large effects sizes were reported for head/trunk stability and reaching, elapsed time, and efficiency (reach/path ratio). Furthermore, these changes were maintained after a 12-week washout period, suggesting that the foundational skills gained from hippotherapy would support occupational performance and participation. Clarke et al. conducted a pilot randomized controlled trial of occupational therapy in patients with Parkinson’s disease (PD).4 Occupational therapy aimed at increasing functional independence and mobility for 2 months was provided in the home to 19 patients. The control group consisted of 20 patients with PD. The trial demonstrated that the Nottingham Extended Activity of Daily Living Scale and the Parkinson’s Disease Questionnaire—39 provide relevant outcomes. Based on the results of this pilot study, to detect significant differences between control and treatment groups (two-tailed p=0.05 with 90% power), the researchers suggested randomizing 750 patients, with 375 for each group. ■ Susan H.
