Young Voices By Erin Klein, DPM It’s Personal I never imagined that my first surgical patient in fellowship would be an eight-year-old boy from Ukraine. He and his mother entered my office one afternoon concerned about a soft tissue mass that had been getting bigger over the past two years. This patient visit represented a situation I had never encountered before. I had never discussed a tumor of the foot with anyone—and particularly not with the mother of a boy who wanted nothing more than to grow up to be a professional hockey player. I decided to leave the brutal honesty at home and try the empathetic, compassionate approach. After I explained that he had a tumor, we discussed treatment options, and she decided she wanted the tumor imaged. After imaging produced no answers and we had a family discussion about the course of treatment, the family decided they wanted the lesion excised. I was okay with that … until I realized we were going to be operating on an eight-year-old. Later that day, my mom called me. I told her about my day and how I hoped I had handled the situation with this patient and his family in an appropriate and professional yet compassionate manner. Rather uncharacteristically for my mom, she was quiet until she told me she was calling with news. Her news? She had a tumor of her own, but it was in her colon, not her foot. Unlike my patient’s tumor, my mom’s tumor was definitely malignant, definitely needed resection, and might require chemotherapy. Going through this process with another family while simultaneously dealing with my mom and her tumor gave me a whole new perspective on compassionate patient care and communication with patients. After that day, I can say without question that I am not a fan of tumors. Regardless of my personal situation, the tumor on my patient’s foot needed to be resected and needed a diagnosis. The day we resected my patient’s tumor, the waiting room looked like a family reunion. Mom, dad, little brother, big sister, two grandparents, an aunt, and two cousins were all waiting for news from me as soon as the little guy woke up. I sincerely wished that I had more to tell them at that point than, “He did great with the anesthesia and the tumor is out.” I realize now how little comfort those words probably provided them. However, it was an amazing feeling when the pathology report came back and I was able to tell them the tumor was not malignant and we did the right thing by removing it. These experiences have made me realize what it really means to treat patients like you would treat your family.
