APMA All Stars Marc Brenner, DPM, Rethinks an Old Problem Marc A. Brenner, DPM, became an avid supporter of his profession in 1971 when he graduated from the New York College of Podiatric Medicine. Since beginning his career as a podiatric physician, he has written countless books, appeared on television programs, and currently has a weekly radio show in Boston. Dr. Brenner lectures around the world and cochairs New Cardiovascular Horizons, a conference that brings together a variety of health-care disciplines for a team approach to cardiovascular care. His goal with all these activities is to “make podiatry famous.” In addition, he has taken part in a new campaign, Rethink Varicose Veins, which supports his goals for the profession and his patients. Like many podiatrists, Dr. Brenner frequently sees varicose veins, or chronic venous insufficiency (CVI). CVI is a progressive disease, not unlike onychomycosis, and if it is left untreated, it can progress to ulcerations, skin changes, or leg swelling. Because these veins are prominent in the lower extremities, podiatrists are often the first medical professionals to see this disease. “Podiatrists are the gatekeepers of wound care,” Dr. Brenner said. “No one sees more feet, and thus legs, than we do.” The underlying causes of CVI are valves in the veins of the leg that no longer work properly, leading to reverse blood flow and ultimately blood pooling in the legs or feet. Traditionally, podiatrists and other physicians used compression and elevation to treat varicose veins. However, there are a variety of new in-office procedures that can address the problem. The role of the podiatrist in educating his or her patients about these new treatment options is critical. All Stars in Podiatric History: Issachar Zacharie Issachar Zacharie was the original “physician to the stars.” He served as chiropodist to many prominent citizens in Baltimore and Washington, DC, in the mid-1800s, then headed west with the Gold Rush to open what was likely the first chiropody office west of the Mississippi River. By the time of the outbreak of the Civil War, Zacharie had returned to the east coast and was treating William Seward, secretary of state. Seward recommended Zacharie’s services to none other than President Abraham Lincoln, who quickly became a patient as well as a friend and confidante. In fact, Lincoln not only took advantage of Zacharie’s medical treatments, but also trusted him to carry top-secret instructions to his military commanders in the south.
