Digest This - Special Edition 2008 - (Page 61) 1951 George Crile Jr., MD, pioneers the subtotal colectomy and ileostomy for toxic megacolon, with marked reduction in operative mortality. 1952 and 1953 Drs. Crile and Turnbull discover the cause of ileostomy dysfunction, which often led to reoperations on patients, and devise a new form of ileostomy to prevent this complication. 1953 Dr. Turnbull accidentally discovers that Karaya powder can be used as a skin barrier for fistulas and ostomies, leading to its use as the standard method of care for the next 20 years. 1954 to 1955 Dr. Turnbull and Walter Furr, MD, develop the first postoperative stoma bag for patients. Dr. Turnbull and John Beach Hazard, MD, discover cancerous cells from colon cancer in portal venous blood, which led to the development of the “no touch” technique for colon cancer resection, with increased patient cure rates compared with standard resection. 1977 The first continent ileostomy procedure is performed at Cleveland Clinic, offering ileostomy patients an alternative to the external pouch. Victor W. Fazio, MD, and Les Bokey, MD, Professor of Surgery, University of Sydney Medical School and head of the Department of Surgery at Concord Hospital, Sydney, Australia, would later modify the continent ileostomy procedure to help prevent the valves from slipping out of place. 1978 Cleveland Clinic surgeon Frank L. Weakley, MD, pioneers the use of a circular stapler in colorectal anastomoses. Today, Cleveland Clinic physicians have the largest experience in the world using a circular stapler for pelvic pouch procedures. Dr. Fazio performs first continent ileostomy at Cleveland Clinic. 1979 David G. Jagelman, MBBS, MS, a staff surgeon in Cleveland Clinic’s Department of Colorectal Surgery, establishes the Familial Polyposis Registry. Today, the David G. Jagelman Inherited Colorectal Cancer Registries, the largest of its kind in the country, helps hundreds of families prevent death from colorectal cancer through patient care, education and research. 1997 Scientists at Cleveland Clinic’s Lerner Research Institute and Allogen Laboratories develop a genetic test to identify a genetic mutation that quadruples the risk of colon cancer among some Jews of Ashkenazi descent. 1995 James M. Church, MD, forms the Collaborative Group of the Americas on Inherited Colorectal Cancer, which was incorporated in 1998. Subsequently, he becomes the first person to hold the office of chairman of two international societies dedicated to the research and treatment of inherited colorectal cancers: The Leeds Castle Polyposis Group and The International Collaborative Group on HnPCC. 1991 Cleveland Clinic surgeon Jeffrey Milsom, MD, performs the first laparoscopic colorectal surgery. 1980s Dr. Fazio performs the first ileoanal pouch at Cleveland Clinic for ulcerative colitis. Cleveland Clinic colorectal surgeons Ian Lavery, MD, and Dr. Fazio develop the stapled pelvic pouch operation, amassing the world’s largest experience in performing this procedure. 2005 Cleveland Clinic’s Department of Colorectal Surgery establishes the International Society of Laparoscopic Colorectal Surgeons. 2007 Cleveland Clinic’s Digestive Disease Center becomes the Digestive Disease Institute. This change is part of Cleveland Clinic’s overall move toward organizing its clinical areas around organ and disease systems, rather than individual specialties. By fully integrating multiple specialties along disease lines, institutes, such as the Digestive Disease Institute, can create a synergy among medical professionals with similar clinical, research and educational interests, centering on patient care. 2008 Dr. Fazio, Feza Remzi, MD, and Bo Shen, MD, of the Digestive Disease Institute, establish America’s first pouchitis/pouch disorder clinic for the treatment some diseases that can affect the pouch. Dr. Remzi and Daniel Geisler, MD, surgeons at Cleveland Clinic and pioneers of a Single-Port Laparoscopic (SPL) method in the field of Colorectal Surgery, perform the world’s first colon resection entirely through a single incision in the navel.
