LETTERS become physicians. I especially enjoyed the interview with Tom Inui, MD, who has been an inspiration to me. I have been concerned about what it means to be a physician all my life. My experiences in medical school, subsequent training, and practice were exciting. Although I felt the forces that tried to limit my caring, I held on to why I became a physician and found a mentor who encouraged me: C. Everett Koop, MD. He encouraged me to become a home care physician. We also worked together on ways to improve the lives of children with special health needs (children with disabilities and/or chronic illness). Dr. Koop and I began private conversations over 30 years ago discussing our mutual concerns about the direction of physician training and practice. In 1999, I invited him to give the keynote address at CHEST 1999, the annual meeting of The American College of Chest Physicians. Instead, he suggested we have a “ﬁreside chat.” He sat at home in front of his ﬁreplace, I sat in the Arie Crown Theatre at McCormick Place in Chicago, and we conducted an historic “telechat” by satellite. This and subsequent conversations led to “The Communication in Health Care Project” of The CHEST Foundation and C. Everett Koop Institute. —Allen I.
