Registering for the big event is easy: There are three easy ways to register in advance. 1. Online at www.gcsaa.org/conference 2. Box 219004 Kansas City, MO 64121-9004 For mail or fax, please complete the registration forms on the following pages. First, select your registration option on Form A-1. Complete Form A-2 for additional seminars and education events. Use Form A-3 to donate to The Environmental Institute for Golf and register your family for trade show floor access. Subtotal all forms and carry fees to Form A-3. Submit all forms with appropriate payment. Only one registrant and his/her family per form. Sorry, but we cannot accept registration by phone. Online registration is available at www.gcsaa.org/conference. On-site registration will open at 6:30 am, Monday, Feb. 2, 2009, in the New Orleans Morial Convention Center. The Golf Industry Show • The Golf Industry Show General Session & USGA Green Section Education Program • Admittance to the trade show all three days: • Thursday, Feb. 5, 11 am – 6 pm (open to all attendees except those registered as family) • Friday, Feb. 6, 9 am – 5 pm • Saturday, Feb. 7, 9 am – 2 pm Complimentary Full-Conference Packages Complimentary registration is offered to GCSAA retired members, inactive members, student members and educator members. As a member benefit, members also receive one complimentary conference registration. New members and others who have never taken advantage of their complimentary registration are invited to do so for the 2009 event. To qualify for complimentary registration, the membership applications of new members must already be approved and their membership dues fully paid. (New members using this option must register by mail or fax only.) Registration Fees (Form A-1) All registrations received on or before 5 pm CST on Thursday, Jan. 15, 2009, will be processed at the advance registration prices located in the first fee column of the Registration Options on Form A-1. Forms received after 5 pm CST on Jan. 15 will be processed at the standard rate in the second column. Any additional funds due, based on this policy, will be payable on site. Media Registration For admittance onto the trade show floor, media personnel must register and show proof as working press or media representative at the time of registration. After registration is complete, media personnel will have access to the conference and show. Seminar Selections (Form A-2) Seminar titles, descriptions, codes and fees can be found in the seminar section of this brochure. Seminar fees are NOT included in any of the registration options. All seminars require an additional fee and have variable pricing. Review the seminar descriptions and indicate your first, second and third choice options on Form A-2. This is necessary because class sizes are limited, and it may be necessary to assign one of your alternate choices. Calculate the amount due for seminars based on your first choices. Course Management/Staff Registration (Mail or fax only) GCSAA members may register their course management and/or staff (as long as they are not members of GCSAA, NGCOA or CMAA) for a full-conference package at their member rate. To register your course management and/or staff, complete a separate form for the person you are registering, select the appropriate registration option, indicate your name and member number in the space provided (below the registration option), and submit the registration forms for everyone at the same time. (To use this option, the forms must be mailed or faxed to GCSAA — online registration is not available.) Family Registration (Form A-3) Complimentary registration is offered to spouses/significant others and dependent children of the main registrant. Family registrants will have access to the trade show floor Friday and Saturday only. Golf Industry Show badges must be worn for admittance onto the show floor. The parent or guardian must sign a Release and Hold Harmless Agreement for children ages 12 and under prior to the children being allowed on the trade show floor. These agreements will be available in the registration area. Golf Course Owner/Employer Registration (Mail or fax only) Golf course owners and employers who are not GCSAA members have the unique opportunity to register for a special conference package for only $175 if their GCSAA member superintendent is registering for a full-conference package. This package allows admittance to all the events included in the GCSAA full-conference package. To register for this option, complete Form A-1, mark the Golf Course Owner/Employer category, indicate your superintendent’s name and GCSAA member number in the space provided, and submit with the superintendent’s registration forms. This offer does not apply to spouses or additional guests. (To use this option, the forms must be mailed or faxed to GCSAA — online registration is not available.) Credential Mailing Credentials will be mailed to all registrants in the United States, Guam, Canada and Mexico for all registrations received and fully paid by 5 pm CST, Tuesday, Dec. 30, 2008. Credentials for registrations received after 5 pm, Dec. 30, 2008, must be picked up in the badge pick-up booth in the New Orleans Morial Convention Center during registration hours. All credentials for registrations from countries other than the U.S., Guam, Canada and Mexico must be picked up at badge pick-up located in the registration area at the convention center. Trade Show Only Registration The trade show only option for attendees, other than distributors, provides access to all three days of the trade show for the following hours: • Thursday, Feb. 5, 11 am – 6 pm (open to all attendees except those registered as family) • Friday, Feb. 6, 9 am – 5 pm • Saturday, Feb. 7, 9 am – 2 pm Cancellation Requests Cancellation requests must be in writing and received by Monday, Jan. 15, 2009, to receive a refund. A $25 administrative fee will be charged for all full and/or partial cancellations. After Jan. 15, no refunds will be made. Special refund note: If your credentials have been mailed at the time of your cancellation, you must return them to GCSAA before any refund will be issued. Please call GCSAA at 800.472.7878 for details. Distributor Registration The distributor registration option is available for those who are distributors of golf, turf or facility products. A copy of your business card is requested at the time of registration. GCSAA members are not eligible for distributor registration unless they are Affiliate (AF) members or Affiliate Corporate Company Representative (AFCR) members. Distribution of Contact Information To facilitate communication between attendees and exhibitors, your contact information, including email address, will be provided to the exhibitors that you permit to scan your badge. In addition, the Golf Industry Show offers pre- and postevent labels, and e-mail addresses are not included in the information provided.
