Integrating Dialectical Behavior Therapy with the Twelve Steps
A Program for Treating Substance Use Disorders
By Bari K. Platter, MS, RN, CNS, and Osvaldo Cabral, MA., C.AC. III
This one-of-a-kind group program incorporates dialectical behavior therapy and Twelve Step Facilitation. Combined,
these widely used, evidence-based therapies provide a dynamic treatment modality to help strengthen a client's
recovery from substance use disorder.
The program works by teaching participants to use a variety of skills in times of distress and to recognize the
usefulness of those skills in recovery. They can practice these skills in groups and on their own, guided by program
worksheets. These learned skills create balanced behavioral patterns that relate to many Twelve Step concepts, which
are referenced throughout the program to help reinforce what is learned.
1. Mindfulness Skills
2. Distress Tolerance Skills
Clients learn skills to quiet the mind and trust their
own perceptions, judgments, and decisions by
practicing and acquiring certain skills.
These sessions teach clients how to successfully cope
with stressful situations and unwanted emotions
instead of doing something that makes things worse.
3. Interpersonal Effectiveness Skills
4. Emotion Regulation Skills
Participants are taught how to communicate more
effectively with other people with the goals of
achieving interpersonal effectiveness, coping with
conflict, keeping relationships, and negotiating
Experiencing and expressing emotions without lashing out,
shutting down, or behaving in a way that is outside of our
spiritual values are the goals of this module while teaching
participants that they have choices other than using
substances to manage their emotions.
Whether implemented in residential treatment settings, intensive outpatient
programs, or in an extended care format, Integrating Dialectical Behavior
Therapy with the Twelve Steps provides everything needed-the guidelines,
session outlines, client worksheets, and handouts-to help professionals
customize an evidence-based program that meets clients' needs.
