UNPRECEDENTED.

This past fiscal year (FY) 2015-16 has been a productive and rewarding year for Health Center
Partners of Southern California (HCP). Our strategic plan, the Program of Work 2020, and the four
key initiatives within that plan guide us in our efforts where we believe we can be of greatest value
to member health centers and the larger health community: 
-Innovation and Thought Leadership;
-Policy, Advocacy and Communication;
-Clinical Care Coordination; and,
-Resource Development.

In advancing each of these initiatives, HCP serves as a nexus for our members and partners to
transform primary care through the power of innovation and collaboration.
One of the biggest transformations we achieved was the successful repositioning of the Council of
Community Clinics as Health Center Partners. Our new name more accurately reflects our commitment
to serving our members as we champion the needs of the safety net population, enhance delivery of
and access to high quality primary care, and serve as a trusted change agent and visionary by fostering
innovative solutions and effective programs that positively impact health center outcomes. 
Our membership expanded in December of 2015 when our board approved for membership
Community Health Systems, Inc. which operates practice sites in San Diego, Riverside and San
Bernardino Counties. As of the end of FY15-16, HCP stands as a premier consortium of 17 primary
care organizations with 126 sites of care and over 768,000 low-income and uninsured patients, a
source for innovative health care programming and services, a new clinically integrated network, and a
flourishing group purchasing organization.
We are growing in many ways: membership, programming, grant revenues, and influence.
This past year furthered many critical initiatives and planted new seeds - successes that will provide a
foundation for the coming year and beyond.
