WORK 2020

Supported our members in Emergency Preparedness
by providing the following types of activities:
facilitated training for member staff on the topic  of
Active Shooter Readiness, participated with members
in the Statewide Exercise, and in Disaster Medical
Surge Planning.

Partnered with the National Association of
Community Health Centers (NACHC) as one of five
Health Center Controlled Networks (HCCN) 
on the Million Hearts Hiding in Plain Site
Initiative, funded by the Centers for Disease
Control and Prevention (CDC). As a result of this
work, the CDC funded a second phase in FY15-16
focusing on hypertension control.
Health Center Controlled Network (HCCN) Data
According to data shared in FY15-16 by John Snow,
Inc. (JSI) that compared clinical performance
between HCCNs on 13 clinical performance
measures, HCP members led the nation in 10 and
equaled other HCCNs in the remaining three. For
example, HCP members topped other HCCNs in
blood pressure control by 2-3 percentage points, in
pap tests by 4-7 percentage points, and in tobacco
screening by  6 percentage points.
