Cath Lab Digest - April 2008 - (Page 52) 52 ACC NEWS APRIL 2008 Healthcare Offerings Help Philips Customers cont. “HeartStart MRx has reduced D2B times, as previously we would have to obtain the 12-lead ECG inside the hospital and wait for a decision on next treatment steps,” said Daniel Wintrow, captain and paramedic with the Glendale Fire Department. “Now, our paramedics are able to send digital quality 12-lead ECG data to the hospital while in transit, which can dramatically cut down on time and help physicians make decisions about appropriate care before arrival.” “With this new capability, we can mobilize resources quickly, better manage and activate the cath lab, and get interventionalists to the hospital for timely and expedited care,” said Dr. Marc Kates, cardiologist, Banner Thunderbird Medical Center in Glendale, Arizona. “By carefully planning and implementing well-defined processes and taking advantage of new technologies, our hospital has experienced reductions in D2B times. We are currently right around the recommended 90-minute guideline, and have seen as much as 44-minute reductions versus our previous averages.” Philips has also announced it will acquire TOMCAT Systems Ltd. based in Northern Ireland. TOMCAT offers a software solution that is able to aggregate, from various sources, data relative to the cardiac care of a patient and presents this in a comprehensive patientcentric view for doctors and nurses. This equips caregivers with a strong tool to help monitor progression of treatment, enable better informed decision making and manage patient care. ■ Abbott’s Xience™ V Drug-Eluting Stent Demonstrates Consistent and Positive Clinical Outcomes Out to Two Years New Data from SPIRIT II Show Continued Low Rates of MACE with XIENCE V D ata presented from Abbott’s SPIRIT II clinical trial demonstrated continued positive clinical results for the Xience™ V Everolimus Eluting Coronary Stent System out to two years, including an observed 40 percent reduction in major adverse cardiac events (MACE) and an observed 44 percent reduction in ischemia-driven target lesion revascularization, TLR compared to the Taxus¨ paclitaxel-eluting coronary stent system. The two-year results from the SPIRIT II trial were presented at the SCAI Annual Scientific Sessions in Partnership with ACC i2 Summit. SPIRIT II is a 300-patient randomized clinical trial, which was conducted in Europe and Asia Pacific to support the launch of Xience V outside the United States. In SPIRIT II, Xience V demonstrated the following key results: • In an analysis of MACE, Xience V demonstrated an observed 40 percent reduction in MACE compared to Taxus at two years (6.6 percent for Xience V vs. 11.0 percent for Taxus). MACE is an important clinical measure of patient outcomes, defined as cardiac death, heart attack (myocardial infarction or MI), or ischemia-driven target lesion revascularization. • Xience V demonstrated an observed 44 percent reduction in ischemia-driven target lesion revascularization compared to TAXUS at two years (3.8 percent for Xience V vs. 6.8 percent for Taxus). • Rates of definite/probable stent thrombosis under the Dublin/ Academic Research Consortium (ARC) definition were 0.9 percent for Xience V and 1.4 percent for Taxus at two years. The ARC definition of latestent thrombosis was developed to eliminate variability in the definitions across various drugeluting stent trials. “In clinical endpoints such as major adverse cardiac events, retreatment and heart attack, patients treated with Xience V in the SPIRIT II clinical trial continue to fare better than patients treated with Taxus out to two years,” said Patrick W. Serruys, MD, PhD, Professor of Interventional Cardiology at Thoraxcentre, Erasmus University Hospital, Rotterdam, and principal investigator of the SPIRIT II clinical trial, who presented the results today. “Xience V is performing as we would expect a next-generation drug-eluting stent should over the long term, specifically on the important clinical endpoint of MACE. The low rate of MACE with Xience V compared to Taxus was present at 6 months, one year and now at two years in the SPIRIT II trial.” In a small subset of patients in the SPIRIT II trial, in-stent imaging results were evaluated at two years. In this 117patient subset, the angiographic in-stent late loss rate was 0.33 mm for Xience V and 0.34 mm for Taxus at two years. Xience V was launched in Europe and other international markets in late 2006. Xience V is an investigational device in the United States and Japan, and is currently under review for approval by the U.S. Food and Drug Administration. Abbott also supplies a private-label version of Xience V to Boston Scientific called the Promus™ Everolimus-Eluting Coronary Stent System. Promus is designed, studied and manufactured by Abbott and supplied as part of a distribution agreement between the two companies. Everolimus is licensed to Abbott by Novartis for use on its drug-eluting stents. ■ Heart Attack Patients Benefit from Thrombus Aspiration Prior to Stenting U sing the Export Aspiration Catheter from Medtronic, before stenting, in patients suffering acute myocardial infarction (AMI) can significantly improve blood flow and survival rates compared to conventional treatment with percutaneous coronary intervention (PCI) alone, according to results of a Dutch study presented at the American College of Cardiology meeting. Presented by Prof. Felix Zijlstra of the University Medical Center Groningen in the Netherlands during a special late-breaking clinical trial session, the study — dubbed TAPAS — found that most AMI patients with ST-segment elevation achieved better reperfusion and clinical outcomes when receiving thrombus aspiration before standard PCI, regardless of their clinical and angiographic characteristics at baseline. It was supported by a grant from Medtronic for angiographic analyses by the core laboratory; all other costs were covered by the Thorax Center of the University Medical Center Groningen. Dr. Zijlstra’s presentation — “Thrombus Aspiration during Coronary Angioplasty for Acute Myocardial Infarction” (2404-5) — sum- marized the design and findings of the study. Researchers at his center randomly assigned 1,071 AMI patients with ST-segment elevation to thrombus aspiration using the Export Aspiration Catheter or to conventional PCI prior to coronary angiography. The Export group had 35 percent fewer patients than the conventional PCI group exhibiting angiographic signs of poor blood flow to the heart muscle.
