Cath Lab Digest - May 2008 - (Page 20) 20 INTERNATIONAL continued from page 1 CATH LAB SPOTL I G H T MAY 2008 King Faisal Specialist Hospital The first cath lab was initially set up in 1984 and the service was expanded to four labs in 1988. Each lab is dedicated for a particular service, but depending on the workload or lab availability, each also has the multi-functionality to be utilized for other specialties. Lab 1 is primarily for pediatric patients, Lab 2 is set up for adults, Lab 3 is our electrophysiology (EP) lab and Lab 4 services both adult and pediatric patients. We have a pediatric recovery room and an adult holding unit, where patients are screened and prepared pre-cath, recovered postcath, and either admitted to the wards or discharged home. We currently have a staff of 34 on our immediate team. This consists of 11 staff nurses (SN1s), 11 cardiovascular technologists (CVTs), 3 radiographers, 5 patient care assistants, 1 secretary, 1 file clerk, a research assistant and our manager. Our longestserving member has been with us since 1987 and 3 of our staff started with us as CVT students in 1991. Our cath lab medical team consists of 4 pediatric cardiologists, 8 adult cardiologists, 2 pediatric and 2 adult electrophysiologists, all of whom are uniquely trained and certified by either North American or European Boards. In 2004, under the umbrella of the KFSH&RC, the King Faisal Heart Institute (KFHI) was inaugurated and today is renowned for being the first cardiac tertiary care center in the Middle East. It is comprised of four clinical sections: adult cardiology, pediatric cardiology, cardiac surgery and cardiac surgical critical care, and delivers medical services to approximately 50,000 patients annually. What type of procedures are performed at your facility? Our scope of service in the cath lab includes: • Diagnostic right and left heart catheterization • Percutaneous transluminal coronary angioplasties (PTCAs), to include: o Balloon angioplasty o Rotational atherectomy o Cutting balloon (Boston Scientific, Natick, MA) o Drug-eluting and baremetal stenting o Thrombus/debris aspiration Aortic stenting Atrial septal defect (ASD) closure Branch pulmonary artery stenting Coarctation dilatation Embolization of collaterals Foreign body removal Intra-aortic balloon pump insertion • Mitral commissurotomy (PTMC) • Patent ductus arteriosus (PDA) occlusion (coil, umbrella and device) • Patent foramen ovale (PFO) closure • Percutaneous pulmonary valve implantation • Pericardial centesis • Pulmonary valvuloplasty • Radiofrequency ablation (for atresia) • Right ventricular (RV) endomyocardial biopsy • Septostomy • Transcatheter fontan completion • Ventricular septal defect (VSD) closure (membranous and muscular) Peripheral interventions are not routinely performed. Our EP lab performs: • All pacemaker procedures from temporary to bi-ventricular implantable cardioverter-defibrillators (ICDs) • Implantable loop-recorder • Cardioversion • Tilt test • 3-dimensional mapping • Ablation of complex arrhythmias (thermo- and cryo-) • Atrial fibrillation ablation We have performed approximately 3,000 procedures in the past twelve months with an average of around 12 cases per day. Thirty percent of our patient population is pediatric and some of our more complex cases may take as many as four hours to complete. Does your cath lab perform primary angioplasty with surgical backup on-site? Yes. The OR desk is informed of any emergency case during our normal working hours and as an OR suite becomes available, it is held by the surgeon on-call until the all-clear is given. Our multidisciplinary teamwork relies on the collaborative skills and services of the Anesthesia, Perfusion Technology, Echocardiology and Respiratory Care Services departments. Elective cases are discussed on a daily basis in a morning meeting with both the cardiologists and surgeons, and if a case is considered to be extremely high risk, an OR may be placed on standby until the procedure is concluded. What procedures do you perform on an outpatient basis? Most elective diagnostic cases scheduled through the pediatric recovery room, day medical unit or the adult cardiac holding unit are performed on an outpatient basis. This includes biopsies and valvuloplasties, but all patients must meet strict discharge criteria before being sent home. All device implantation procedures require an overnight admission with
