Supply usage is closely monitored by our inventory database and each low-volume item has a cath lab staff member assigned to it for ongoing inventory counts and reorder alerts. To avoid overstocking or expiry, some orders are placed on a consignment basis. Has your cath lab recently expanded in size and patient volume, or will it be in the near future? This year has seen a steady growth of 10% in our patient numbers over the previous year and our service has been expanded even further with the screening, admission and discharge of pediatric patients through our recovery room. Is your lab involved in clinical research? The King Faisal Specialist Hospital has its own dedicated research center which is housed in a four-story, technologically-oriented building covering an area of 140,000 square feet that has been architecturally and organizationally integrated with the hospital. The cath lab itself and its cardiologists are continually involved in research projects, publications and registries. Updates to this effect can be found at http://www.kfshrc.edu.sa/KFHI We are proud of having the only active heart-transplant program in the Middle East. Projects ongoing from 2006 include: • Optimization of Protection in Pediatric Cardiac Catheterization Interventional Radiology – This study aims to identify the procedures that give an estimated dose approaching the threshold value of 2 Gy for skin erythema, compare the estimated doses with published data and recommend dose reduction methods. Chlamydia Pneumoniae Deoxyribonucleic Acid (C. Pneumoniae DNA) & Coronary Artery Disease: A Pilot Study – The results of this prospective, randomized pilot-study may help us to understand the underlying inflammatory process associated with atherosclerosis and C. Pneumoniae. Sodium Bicarbonate in Preventing Contrast Induced Nephropathy (SIPCIN): A Randomized Controlled Study – A comparative study of the effects of normal saline versus sodium bicarbonate on renal impaired patients undergoing catheterization. Better Outcomes After Intracoronary Eptifibatide – Intracoronary bolus administration of eptifibatide may provide better immediate and intermediate outcomes than intravenous administration. Fontan completion without surgery – There are several modifications introduced in the preparation for a subsequent non-surgical transcatheter completion of the Fontan procedure. We report our experience with one type of the modification and the short-term results following its implementation. Percutaneous Ventricular Septal Defect (VSD) Closure: The KFSH&RC Experience – The aim of this retrospective review was to further examine the safety and effectiveness profiles of the Amplatzer device used in VSD closing. Effect of Percutaneous Coronary Intervention (PCI) on Diabetic Patients – This retrospective study is analyzing the effect of PCI in diabetic patients with coronary artery disease. The analysis includes an assessment of the status of the coronary artery stents, the rate of restenosis, progression of neo-atherosclerosis factors affecting regional wall motion and ejection fractions. • Permanent Pacing in Pediatric Patients: the King Faisal Specialist Hospital Experience – This is a retrospective review to evaluate the experience and long term results of pacemaker therapy in children treated at the KFSH&RC. Trials: • E-Select Registry – This is a multi-center observational study (500 centers worldwide) will collect national profiles and epidemiological data on patients who received the Cypher Sirolimus-Eluting Stents (SES) (Cordis Corp., Miami, FL). • PROTECT Trial – This is a prospective, multi-center, randomized, two-arm, open-label trial of patient related outcomes with Endeavor (Medtronic, Inc., Santa Rosa, CA) versus Cypher stenting. Registries: • Percutaneous Transluminal Coronary Angioplasty (PTCA) Registry – This is an ongoing registry of patients who underwent PTCA at the KFSH&RC. The objective is to examine revascularization strategies for coronary artery disease and the outcomes of interventions for patients with acute coronary syndrome and chronic coronary insufficiency. • Mitral Balloon Valvotomy Registry – This registry includes short, intermediate and longterm follow-up data on patients who underwent a mitral balloon valvotomy procedure. • Pediatric Heart Catheterization Registry – The aim of this project is to establish a registry for all diagnostic and interventional pediatric cardiac catheterization performed at the KFSH&RC. • Congenital Heart Disease Registry – This registry is a collaborative project between the KFHI and the biostatics, epidemiology and scientific computing department at the research centre. Have you had any cath lab-related complications in the past year requiring emergent cardiac surgery? Yes. With the complexity of our cases, it is not entirely uncommon that surgical intervention may be required in some instances. What other modalities do you use to verify stenosis? Stenotic lesions may be further evaluated by intravascular ultrasound (IVUS), the pressure guidewire system (for fractional flow reserve measurement) and post-processing quantification. We have recently installed cardiac 3D software technology for reconstruction of diseased vessels in three dimensions. What measures has your cath lab implemented in order to cut or contain costs? This is a global problem and not so easily contained, because of our location in Saudi Arabia and our reliance on overseas vendors.
