We perform diagnostic caths, cardiac and vascular interventions, EP studies, ablations, pacemakers and implantable cardioverter defibrillator (ICD) procedures. We have recently started doing atrial septal defect (ASD) and patent foramen ovale (PFO) closures and alcohol septal ablations. We also recently started doing a few peripheral interventions, mostly iliac and renals. Our annual procedure volume is approximately 9,600 procedures. Does your cath lab perform primary angioplasty with surgical backup on-site? Our percutaneous coronary intervention (PCI) procedures are usually performed with a “next available room” type of surgical backup. On rare occasion, for very high-risk patients we may ask for full surgical backup. This would be scheduled by making multiple phone calls — to a CV surgeon, cardiovascular surgery, and anesthesia. What procedures do you perform on an outpatient basis? Diagnostic caths, pacemaker and ICD generator changes, and EP studies. What percentage of your patients are female? 47%. What percentage of your diagnostic cath patients go on to have an interventional procedure? 37%. Has your cath lab recently expanded in size and patient volume, or will it be in the near future? We increased our volume 10% over the last year, and expect to increase more when we open our vascular lab in November of this year. Who manages your cath lab? Justine Childers, RN is our cath lab manager, and she takes care of the day-to-day operations. She has worked in cardiology since 1998, and has been in the cath lab three years. She reports to the cardiovascular laboratories director. Do you have cross-training? Who scrubs, who circulates and who monitors? Our x-ray technologists scrub and usually pan for the physicians. They also do most of the hemodynamic measurements and the supply documentation. RNs scrub, circulate and monitor the patient, and do the procedure documentation. Our RNs have started cross-training in what has been our typical RT role. We think this will give us a lot more flexibility and enhance our ability to get our procedures done even more efficiently. Does an RT have to be present in the room for all fluoroscopic procedures in your cath lab? No, but there does have to be at least one in the department. Which personnel can operate the x-ray equipment (position the II, pan the table, change angles, step on the fluoro pedal) in your cath lab? Our RTs and some of the RNs do all of the above. If your lab performs peripheral interventions, what disciplines are involved? Our cardiologists are the only physicians who use our lab for peripheral procedures. How did your lab begin performing peripheral cases? One of our cardiologists completed his fellowship with vascular experience, and some of our current cardiologists went to training courses so that they would be able to perform vascular procedures. They must go through our hospital’s credentialing process in order to do these procedures. What specific equipment was instituted and/or dedicated towards peripheral cases above and beyond what is used for coronary cases? Any helpful tips? We’re not there yet, so we are the ones that could use the helpful tips! I dread the coding more than anything.
