A prospective, multi-center, randomized study of aqueous oxygen (AO) therapy for 90 minutes in anterior AMI patients with successful reperfusion (via PCI) < six hours after symptom onset. The objective of the study is to determine whether intracoronary perfusion of hyperoxemic blood in the AO therapy group immediately after successful PCI/ stenting for the treatment of acute myocardial infarction reduces the area of infarction (% left ventricle) as measured by T c99m Sestamibi SPECT imaging at 14 days post-PCI, with no worse than a six percent increase in the incidence of major adverse cardiac events. Randomized, controlled trial of the Medtronic endeavor drug-eluting (ABT-578) coronary stent system versus the Cypher sirolimus-eluting coronary stent system in de novo native coronary artery lesions. Primary Investigator: Aaron Kugelmass, MD • ENDEAVOR IV. A randomized, controlled trial of the Medtronic Endeavor drugeluting (ABT-578) stent system versus the Taxus paclitaxel-eluting coronary stent system in de novo native coronary artery lesions. This study is investigating the utility of a new drug-eluting stent of the treatment of de novo or restenotic coronary artery lesions or vessels experiencing abrupt or threatened closure. Primary Investigator: Aaron Kugelmass, MD • TIMI-38. A comparison of CS-747 and clopidogrel in acute coronary syndrome subjects who are to undergo percutaneous coronary interventions. Primary Investigator: Aaron Kugelmass, MD • TAXUS-ATLAS. Treatment of de novo coronary disease using the TAXUS Liberte-SR paclitaxel-eluting stent. The purpose of this study is to evaluate the safety and efficacy of the TAXUS Liberte-SR stent in the treatment of de novo lesions compared with the TAXUS express paclitaxeleluting coronary stent system. Adam Greenbaum, MD Have you had any cath lab-related complications in the past year requiring emergent cardiac surgery? We have had one case in the past two years. What other modalities do you use to verify stenosis? Are you using physiology for lesion assessment? We primarily use FFR, fractional flow reserve, to determine stenosis severity in intermediate lesions. The PressureWire (Radi Medical Systems, Wilmington, MA) measures a gradient With advances in treatments for patients with congenital and acquired cardiac diseases, and rapid advances in noninvasive cardiac imaging such as CT and MRI angiography, I envision significant changes in both the types of patients and types of procedures performed in adult catheterization laboratories over the next decade. In diagnostic laboratories, I envision a shift toward more complex hemodynamic studies for survivors of congenital heart disease and those with complicated physiology, alongside a more streamlined approach to coronary angiography directed toward patients already selected for percutaneous revascularization. And, as percutaneous interventional technology and techniques advance, I envision a wider spectrum of less invasive and novel treatments for pathology such as long chronic total occlusions, valvular heart disease and heart failure, not previously imagined to ever be percutaneously approachable. — Adam Greenbaum, MD, Associate Director Cardiac Catheterization Lab
