A HEALTHY START LETTER FROM THE CEO REGIONAL PARTNERSHIP Health care in our state and across the country continues to evolve and change to provide the best possible care at the right time and in the right place. As a member of the University of Maryland Medical System, we are proud to be a part of something greater. Greater access to physicians and specialists as well as a network of forward-thinking health care champions allows us to continue our vision to create the healthiest community in Maryland. Just recently, our hospitals and Union Hospital of Cecil County joined other organizations such as Healthy Harford, the Harford County Health Department, and the Office on Aging (and others) in proposing a strategy that I believe will transform health care in our region for the better (see page 18). Using available technology and a unified approach, we will provide a more efficient and proactive method to serving our community in need. We were one of 22 coalitions that applied for grants to support new approaches to the changing health care landscape. Last spring, our group was awarded $2.7 million a year for the next four years to implement our program. Using an integrated group of nurses, social workers, and other community health workers, we will work together to provide coordinated health care and social resources to Medicare patients in their homes and community who are at high risk for serious health complications. This program builds from the successes that we have been testing for almost two years. The goal for all of us is to improve quality of life through support and care of our more vulnerable neighbors.
