The major difference with other operations of the same kind is that the gastric volume is actually reduced with the sleeve gastrectomy, whereas in earlier versions, like vertical banded gastroplasty and adjustable band gastroplasty, the total volume of the stomach remained unchanged and the restricted volume concerned only a gastric pouch connected to the rest of the stomach via a small “stoma.” There is some evidence that sleeve gastrectomy does more than just to restrict the gastric volume. Melissas and, before him, Carmichael showed that gastric emptying was accelerated after sleeve gastrectomy, which could explain a certain dumping effect T experienced by some patients. The first short-term results of this procedure were encouraging as they approached the results of Roux-en-Y gastric bypass. The aim of this article is to evaluate midterm results. METHODS In our department of bariatric surgery, all patients are evaluated and treated according to an empirically established algorithm. In brief, all patients who do not experience gastroesophageal reflux, are not diabetic, and are not classified as sweet eaters are treated by a restrictive procedure. As of October 2001, the restrictive procedure of choice in our center has been a sleeve gastrectomy.
