Patel, MD; Tomas Escalante-Tattersfield, MD; Samuel Szomstein, MD, FACS; and Raul Rosenthal, MD, FACS The Bariatric Institute, Section of Minimally Invasive Surgery, Cleveland Clinic Florida, Weston, Florida Abstract: Jejunoileal bypass (JIB) for the treatment of obesity was first described in 1969.1 The procedure consisted of connecting the proximal jejunum to the distal ileum, bypassing most of the small bowel.1 The resulting malabsorption led to considerable weight loss and gained great acceptance as a treatment for severe obesity. Eventually, the severe metabolic side effects of malabsoprtion exceeded the benefits of weight loss.2 To date, the JIB is no longer recommended as a bariatric procedure.3 Several authors have reported surgical reversals of JIB.2,4-6 We present the case of a patient who had a JIB reversal at our institution. After reversal, however, the patient became morbidly obese over a short period of time, requiring a new bariatric procedure. The patient’s history of life-threatening complications from previous chronic malabsorption led to the selection of a restrictive procedure, the sleeve gastrectomy. This is the first report of a sleeve gastrectomy performed after the reversal of a JIB.
