Dear Readers, In trying to stay up to date with my editorials, I came across a very interesting article published in the August 2008 issue of Nature that I thought I must share with you because of the question the article brings to mind. Raul J. Rosenthal, MD, FACS, Clinical Editor, Bariatric Times, Program Director of Minimally Invasive Surgery, Director of the Minimally Invasive Fellowship Program, and Director of the Bariatric and Metabolic Institute, Cleveland Clinic Florida—Weston, Fort Lauderdale, Florida. born with a good supply of brown Is there such a thing as good fat and bad fat? The answer is an astonishing YES. Researchers at the Dana Farber Cancer Institute and Cell Biology at Harvard Medical School, headed by Dr. Bruce Spiegelman, showed that there are “brown fat cells” and “white fat cells.” The brown fat cells are filled with mitochondria and iron and are responsible for BURNING CALORIES AND RELEASING ENERGY. White fat cells instead are simple bags that store energy and CONTRIBUTE TO OBESITY and disease. Though humans are fat cells, they disappear after infancy. This information, in turn, raises several questions. Do we become obese due to the lack of brown fat cells or abundance of white fat cells? If so, can we promote brown cell fat or eliminate white cell fat? Dr. Spiegelman and his team are ahead of the game in discovering that brown fat cells derive from skeletal muscle cells. More precisely, they conclude that they derive from myoblastic cells that express the Myf5 gene, a marker thus far found only in muscle cells. Further investigation done by this group shows that PRDM16, a zinc-finger protein known to stimulate brown fatselective gene expression, can powerfully regulate the bidirectional cell fate switch between muscle and brown fat. As a dominant regulator of the brown fat cell fate, PRDM16 may have POTENTIAL USE THERAPEUTICALLY IN OBESITY. Isn’t this a breakthrough in the STRUGGLE TO UNDERSTAND WHY SOME OF US ARE OBESE and others are not? Can you imagine doing brown fat cell transplantation? I encourage you to read this fascinating article and follow Dr. Spiegelman’s research, which I am sure will become a key aspect in understanding the ethiology, pathophysiology, and treatment of the disease of obesity. Please take some extra time to review this issue of Bariatric Times, which contains several articles on surgery, as well as our regular Consultant’s Corner and an extremely interesting paper on hair loss and the bariatric patient. With an article on EMERGING TECHNOLOGIES FROM OUR ASMBS PRESIDENT, DR. SCOTT SHIKORA, an article on the NEW ROSE PROCEDURE, and a case report on SLEEVE GASTRECTOMY FOLLOWING JEJUNOILEAL BYPASS REVERSAL, you are sure to be prepared for the upcoming meetings this fall. Hope to see you in Buenos Aires at IFSO or in San Fran at ACS! Enjoy, Matrix Medical Communications 4975 West Chester Pike PO Box 445 Edgemont, PA 19028-0445 © 2008 Matrix Medical Communications.
