Synopsis: Researchers conducted a prospective, double-blind study to evaluate and compare the effects of laparoscopic Roux-en-Y gastric bypass with laparoscopic sleeve gastrectomy on body weight, appetite, fasting, and postprandial ghrelin and peptide-YY levels in 32 patients, half of whom were assigned randomly to each technique. They found markedly reduced ghrelin levels and increased peptide-YY levels after laparoscopic sleeve gastrectomy, and conclude that these results are associated with greater appetite suppression and excess weight loss in laparoscopic sleeve gastrectomy compared to laparoscopic Roux-en-Y gastric bypass. They conclude that laparoscopic sleeve gastrectomy was a safe and effective option for these patients and may be considered as a stand-alone technique. Synopsis: Authors discuss the results of laparoscopic sleeve gastrectomies performed for 63 super-super-obese patients (BMI>60kg/m2) and compare these results with those of laparoscopic adjustable gastric banding performed in similar patients. They found that laparoscopic sleeve gastrectomy resulted in short-term weight-loss comparable to that of laparoscopic adjustable gastric banding, but conclude that laparoscopic sleeve gastrectomy requires less reoperation because foreign material is not implanted in the body and because the residual ghrelin-producing gastric mass is smaller.
