Psychiatry 2008 - September 2008 - (Page 53) [journal watch] population-based study. Schepis TS, Krishnan-Sarin S. J Am Acad Child Adolesc Psychiatry. 2008 Jul;47(7):745-754. Synopsis: Data from 18,678 adolescents (ages 12-17) were used to examine risk factors associated with misuse of opioids, stimulants, tranquilizers, and sedatives and to characterize the symptoms of abuse and/or dependence. Researchers found that 8.2% of the adolescents misused a medication and 3.0% reported symptoms of substance use disorder related to misuse of prescription medication within the past year. Predictors of misuse were poorer academic performance, major depression in the past year, higher risk-taking levels, and use of alcohol, cigarettes, marijuana, cocaine, or an inhalant in the past year. Risk factors associated with symptoms of dependence were major depression in the past year, use of cocaine or an inhalant in the past year, and 10 or more episodes of prescription misuse in the past year. PMID: 18520963 Cannabis withdrawal is common among treatment-seeking adolescents with cannabis dependence and major depression, and is associated with rapid relapse to dependence. Cornelius JR, Chung T, Martin C, et al. Addict Behav. 2008 Feb 11. [Epub ahead of print] Synopsis: To test (1) whether cannabis withdrawal is common among adolescents and young adults who are comorbid for cannabis dependence and major depression, and (2) whether the presence of cannabis withdrawal symptoms are associated with rapid relapse, researchers analyzed data from a total of 170 comorbid adolescents and young adults (104 with cannabis dependence, 32 with cannabis abuse, and 34 with cannabis use without dependence or abuse). They found that cannabis withdrawal was present in 92% of the cannabis-dependence group, and was associated with rapid relapse in the total group. Researchers conclude that cannabis withdrawal should be a criterion for diagnosis of cannabis dependence. PMID: 18313860 women with histories of selfharming behavior. Emergent themes discussed include great expectations, I speak but no one hears, sexual naivety meets sexual violence, and redrawing the sexual map. PMID: 16181154 BENZODIAZEPINES AND TREATMENT OF ANXIETY Combination treatment with benzodiazepines and SSRIs for comorbid anxiety and depression: a review. Dunlop BW, Davis PG. Prim Care Companion J Clin Psychiatry. 2008;10(3):222-228. Synopsis: Authors conducted a literature review to examine (1) the co-occurrence of anxiety with depressive disorders in adults, and (2) the rationale for and use of combination treatment with benzodiazepines and selective serotonin reuptake inhibitors/serotonin-norepinephrine reuptake inhibitors (SSRIs/SNRIs) for treating comorbid anxiety and depression. They found, among other things, that comorbidity is common among patients with anxiety and depressive disorders, with subsyndromal symptoms often present even when full criteria for both separate disorders are not met. They also found that benzodiazepines seem to improve both response speed and overall response. Authors give recommendations for long-term management plans. Scourfield J, Roen K, McDermott L. Health Soc Care Community. 2008 May;16(3):329-336. Epub 2008 Mar 3. Synopsis: Authors examine the cultural context of youth suicide and connections between sexual identity and self-destructive behavior in 69 lesbian, gay, bisexual and transgender young people using focus groups and interviews. They also describe strategies that this group uses when stressed, and categorize them as resilience, ambivalence and self-destructive behavior, including self-harm and suicide.
