Synopsis: In this review of the efficacy, effectiveness and dropout for adverse effects related to use of benzodiazepines, the author found no convincing evidence of shortterm effectiveness while adverse effects were 1.5-times more frequent in drug-treated versus placebo patients. The author presents explanations for these results, illustrates changes in diagnostic criteria over time, and discusses lack of accepted methods of assessing estimates of effectiveness in clinical practice. Based on the results, they conclude that sustained D(2) occupancy at or above the accepted threshold with acute clinical response may not be necessary to maintain response, a finding that they conclude has clinical implications for antipsychotic dosing and development. Synopsis: Researchers monitored long-term symptomatic tolerability and remission in patients with stable but suboptimally treated psychoses after switching to risperidone long-acting injectable (RLAI) in 529 patients who were followed for up to 18 months. Based on the results, authors conclude that (1) RLAI is safe during longterm treatment up to 18 months in adults requiring antipsychotics, (2) conversion to RLAI resulted in improved symptom control, and (3) most patients achieved and maintained a sustained remission of 6 months or more after conversion to RLAI. Synopsis: Author reviews longacting injectable risperidone for the treatment of schizophrenia, including background on depot formulations of conventional antipsychotics, and a discussion of two 12-week, double-blind, randomized, phase III studies and one open-label, 12-month study. Based on the findings, author concludes that the safety and tolerability of the long-term formulation of risperidone are comparable to those of the oral formulation, the local tolerability at the injection site is good, and that consideration for a broader use of the long-term formulation is warranted.
