Synopsis: To examine differences among the cholinesterase inhibitors donepezil, galantamine, and rivastigmine for the treatment of Alzheimer’s disease, authors conducted a review of the literature. They found no statistically significant differences among drugs with regard to cognition, but found the relative risk of global response better with donepezil and rivastigmine than with galantamine, behavior was better with donepezil compared to galantamine, and incidence of adverse events was generally lowest for donepezil and highest for rivastigmine. Synopsis: To evaluate the efficacy, safety and tolerability of rivastigmine capsules in patients diagnosed with probable vascular dementia, researchers randomized 710 vascular dementia patients (50–85 years of age) to rivastigmine capsules (3–12mg/day) or placebo and measured global and cognitive performances, activities of daily living, neuropsychiatric symptoms, and adverse events. They did not find that rivastigmine provided consistent efficacy in probable vascular dementia, and apparent efficacy on cognitive outcomes was derived from effects in older patients likely to have concomitant Alzheimer pathology.
