Your guide to As a Cleveland Clinic patient, you have access to MyChart, the secure, online tool that connects you to your personalized health information from the privacy of your home at any time, day or night. MyChart also provides important, health-related reminders to help you better plan your ongoing healthcare. All you need is Internet access and an email account. We want you to be actively involved in every aspect of your healthcare. MyChart connects you to important healthcare information, at your convenience. Here’s a snapshot: Reminders We can send a message to your MyChart account if you have a Health Maintenance item—such as a mammogram or blood work—listed as “due soon” and are being followed by a primary care physician of Cleveland Clinic. The message will contain the instructions needed to schedule your appointment. Letters Letters composed by your healthcare team can be viewed and printed from MyChart. You have access to the letter the day it is prepared. Health Trends MyChart users can view vital signs taken during their last 15 ofﬁce visits. You can view pulse, blood pressure, weight and respirations. With a simple push of a button, you can view them by graph or by date. Test Results View details of your test results and read any comments added by your physician. You can chart and graph numeric test results or print copies to easily compare past results. Immunization MyChart allows you to view and print a record of your immunizations for school, work, etc. It also includes the vaccine statement from the Centers for Disease Control to learn more about the vaccines. Prescription Renewal Request a renewal of a prescription. If you have no reﬁlls left for a prescription, you may want to request a renewal from your physician. For your convenience you will receive a MyChart message when the renewal has been processed and is ready for pickup. Appointment Request Established patients also have the ability to request or cancel appointments. It’s as simple as selecting your provider from a pull-down menu, choosing a preferred date and time range and entering a reason for your visit. A confirmation message will be sent when the appointment is scheduled. 6 HEALTH ESSENTIALS “I really appreciate the convenience of MyChart!
